ARTICULAR INJURY OF THE WRIST PDF

Garcia-Elias, M. - Mathoulin, C.

Hand and wrist injuries account for millions of emergency room visits each year. Although the most frequent type of articular wrist injury involves the distal radius, there are many other fractures that require skillful intervention to obtain maximum, long-term function. This book focuses on those complex intra-articular wrist injuries that have not been extensively covered in the past, providing a complete picture of their clinical, radiographic and therapeutic features. It is essential for all specialists who are regularly confronted with these challenging injuries. Special Features:Each chapter written by a renowned international specialist who has pioneered significant advances in the field in the last five yearsPresents a detailed review of surgical approaches to acute intra-articular wrist injuries, ranging from partial ligament tears to the most complicated dislocationsProvides an overview of basic wrist anatomy, biomechanics, injury classification, clinical presentation, assessment and diagnosis, and treatment recommendationsIncludes discussions of the most promising minimally invasive treatments, especially arthroscopically guided fracture reductionHighlights all concepts in nearly 500 instructive, beautifully drawn surgical diagrams, radiographs and intraoperative photographs Packed with practical treatment guidelines, Articular Injury of the Wrist is invaluable for all hand surgeons as well as for orthopedic and plastic surgeons and fellows. It will quickly increase their knowledge of those difficult wrist injuries that have not been well documented previously, but are no less devastating in terms of early joint degeneration and dysfunction if not treated properly. Contents 1. Introduction 2. Anatomy of the distal radius 3. Surgical approaches to the wrist joints 4. Incidence and pathomechanics of carpal injuries 5. Imaging of the articular injury of the wrist 6. Trapezium, trapezoid and capitate fractures 7. Hamate and Hook of the Hamate fractures 8. Pisiform fractures 9. Proximal row fractures (scaphoid and Kienböck’s excluded) 10. Undisplaced scaphoid fractures 11. Displaced scaphoid waist fractures 12. Scaphoid proximal pole fractures 13. Perilunate dislocations 14. Trans-scaphoid perilunate dislocations 15. Scapho-capitate fracture-dislocation 16. Fracture-dislocations of the wrist, other than perilunate 17. Partial tears of the scapholunate ligaments 18. Arthroscopy treatment of scapholunate ligament disruptions 19. Open treatment of scapholunate ligament disruptions 20. Lunotriquetral dissociation 21. Radiocarpal fracture-dislocation 22. Intraarticular fracture of the distal radius (AO types B-C1-C2) 23. Intraarticular fracture of the distal radius (AO type C3) 24. Unstable ulnar styloid fracture 25. Natural History of traumatic TFCC tears 26. Triangular fibrocartilage tears 27. Galleazzi’s fracture-dislocation 28. Complex open fracture-dislocations of the wrist: ARTICULAR INJURY OF THE WRIST.

In one series of intra-articular fractures, the likelihood of a concomitant soft-tissue carpal injury was nearly 50%. 30 Intra-articular involvement of the radiocarpal joint or DRUJ may occur resulting in loss of joint surface integrity. Injury to the TFCC is often heralded by an associated fracture of the ulnar styloid.

Wrist osteoarthritis is a group of mechanical abnormalities resulting in joint destruction, which can occur in the wrist. These abnormalities include degeneration of cartilage and hypertrophic bone changes, which can lead to pain, swelling and loss of function. Osteoarthritis of the wrist is one of the most common conditions seen by hand surgeons. 11/06/2013

Articular cartilage injury at the beginning stage is called as “focal chondral defect”. If proper treatments are not given at this beginner stage the Articular cartilage injury would become worse very soon. If the Articular cartilage injury is ignored it becomes long term or chronic and could lead to the disease of osteoarthritis.
